It is also known as black fungus; it can turn dangerous if left untreated. Doctors said on Friday amid reports of re-emergence of the rare deadly fungal infection among COVID-19 patients across hospitals in Mumbai, Delhi, Pune, and Ahmedabad.

It is caused by a fungus called Mucorales, which is usually not known to affect humans, it causes severe damage to patients during both treatment and after recovery, often necessitating urgent surgery to remove affected parts above the neck, according to experts.

“The problem was not witnessed on such a big scale during the first Covid wave. But this time around, it has gone up multifold all over the country. Normally, we don’t have more than one or two such patients requiring surgeries in a year. But I had to do over 30 such surgeries in the last two months,” said eminent ENT surgeon Madan Kapre.”

“Covid patients get heavy doses of steroids and antibiotics during hospitalization, causing a dip in immunity levels. Those with diabetes have sugar levels several times higher than normal, which severely compromises their immunity. So, many such patients get fungal infections both during the infection and after recovery. The infection affects several parts of the body like the nose, eyes, and even the brain, creating a gangrene-like situation and thus requiring the removal of affected parts by surgery. I have removed the eyes of about 15 patients so far,” he added.

SYMPTOMS OF MUCORMYCOSIS:

The common symptoms associated with the disease include headache, facial pain, nasal congestion, loss of vision or pain in the eyes, swelling in cheeks and eyes, and black crusts in the nose. The US Centre for Diseases Control and Prevention (CDC) estimates Mucormycosis with an overall all-cause mortality rate of 54 percent.

TREATMENTS OF MUCORMYCOSIS:

Intravenous (IV) amphotericin B (a lipid formulation) is the drug of choice for initial therapy [108,109]. Posaconazole or isavuconazole is used as step-down therapy for patients who have responded to amphotericin B. Posaconazole or isavuconazole can also be used as salvage therapy for patients who don’t respond to or cannot tolerate amphotericin B; for salvage therapy, the decision to use oral or IV posaconazole or isavuconazole depends on how ill the patient is, whether an initial course of amphotericin B was able to be administered and whether the patient has a functioning gastrointestinal (GI) tract.

Surgery

Aggressive surgical debridement of involved tissues should be considered as soon as the diagnosis of any form of mucormycosis is suspected. Surgical intervention with removal of necrotic tissue and debulking infection has been associated with improved survival in anecdotal clinical reviews of rhinocerebral and pulmonary infection.

Early initiation of antifungal therapy improves the outcome of infection with mucormycosis. This was illustrated in a retrospective study of 70 patients with hematologic malignancy who had mucormycosis in which delayed amphotericin B therapy (starting treatment ≥6 days after diagnosis) resulted in an almost twofold increase in mortality at 12 weeks after diagnosis (83 versus 49 percent).
